This class discussion board focuses on assessing the characteristics of programs delivered by human services organizations. Identify your human service organization and a specific service / program. If you don’t work for a human services organization, select one that you might use for your final project. It may be one with which you are familiar or one from whom you or your family have received services. You may have to do some research to understand the organization. Briefly discuss the intended strategic outcome for the service (an outcome describes the changes in behaviors / skills of individuals and/or conditions for individuals, groups, or communities) and how the organization might measure the results of the service.

I have selected Friends Village at Woodstown (Nonprofit Continuing Care Retirement Community) as my human service organization and their Generation to Generation Mentoring Program as my discussion topic. This cross-generational, socialization effort is run by the facility’s Life Enrichment Director and department staff members. Ten students from a local middle school visit with ten Independent Living residents. The same matched pairs meet twice a week for one-hour sessions during the school year. The group gathers in the Friends Village auditorium for 30 minutes of guided activity and then 30 minutes of free activity.Weekly sessions include opportunities for physical fitness and other activities such as arts/crafts and board games.

Generation to Generation was designed to have a positive impact on the seniors (adults over age 62) and the middle school children.Identified outcomes for the seniors are: 1.) to increase socialization with the younger generation, 2.) to decrease time spent in isolation for seniors, and 3.) to increase physical activity. Intended outcomes for participating youth are: 1.) to increase socialization with the older generation, 2.)to decrease screen time (television, smart phones, video games), and 3.) to increase physical activity.

This program was created to decrease isolation and physical inactivity for both populations as well as decrease time spent engaged in electronic activities for youth. Exercise programs and socialization opportunities for older Americans help prevent many of the problems that isolated seniors experience, like depression, memory loss, and chronic disease. (The Importance of Elderly, 2011). When youth interact with older adults, their egocentric tendencies change, and they begin to recognize the needs and interests of others.In addition, this program also connects both populations to a different generation, yielding a better understanding of the experiences and backgrounds of both groups.

Outcomes are measured by the collection of surveys distributed by Friends Village staff and data from an electronic check-in kiosk. Pre and post surveys, in the form of two evaluation questionnaires are completed by each youth and senior at six and twelve-month intervals. The electronic kiosk determines how much time each participant spends in physical activity on a weekly basis. A third-party representative, The Salem Health and Wellness Foundation, will review all data to determine the effectiveness of the program at the six and twelve-month intervals.

The Division of Substance Abuse and Mental Health (DSAMH) has a program, PROMISE, which serves as a road to recovery for individuals with substance abuse and mental health conditions. PROMISE stands for Promoting Optimal Mental Health for Individuals through Supports and Empowerment. The goal of PROMISE is to provide individually-tailored, community-based, and recovery-oriented services to help people live independently in the community and avoid unnecessary institutionalization (Department of Health and Social Services (DHSS), 2016). The program provides care management, where there is ongoing monitoring of the provision of services which is included in the recipient’s recovery plan. Part of the care management includes quarterly monitoring and annual assessments; necessary changes and referrals are made on a quarterly basis during monitoring or as needed. During annual assessment, the level of care for an individual is evaluated; results could be an individual gets moved up to a higher level of care, moved down to a lower level of care, or discharged to outpatient due to having met all treatment goals and no longer needs the services (DHSS, 2016). The strategic outcome is measured by DSAMH Performance Improvement Committee, who in collaboration with care management staff, “collects data using various instruments, tools and checklists. Data is collected monthly and analyzed on a quarterly basis using discovery and remediation” (DHSS, 2018).

The importance of evaluating and measuring a program is that it shows the progress of the service being provided and whether or not the pre-established goals have been met. This will allow an organization to address issues that may arise or potential barriers that could hinder the service (United States General Accounting Office, 1998).
